Abstract: 

Background and purpose: Oculogyric crisis which is a dystonic reaction commonly caused by typical antipsychotics treatment of acute dystonias, rarely occurs with atypical antipsychotics like clozapine. The aim of this report was to describe a rare case of clozapine-induced oculogyric crisis successfully treated with biperiden. Case: The patient was a 42-year-old woman with a 19-year history of schizophrenia treated with high-potency typical antipsychotics, risperidone and lithium augmentation with appropriate doses and duration. Because of poor response to prior medications, she was prescribed clozapine started with 25 mg/d and gradually titrated up to 150 mg/d. She has experienced two episodes of oculogyric crisis treated successfully with biperiden. Discussion: Although low affinity of clozapine for blocking D2 dopaminergic receptors and its anticholinergic activity may explain the significantly lower incidence of dystonic reactions, they may happen in some patients and could be suppressed with biperiden. Conclusion: Clinicians should consider dystonic reaction as a possible clozapine adverse effect and on the basis of this report, clozapine-induced oculogyric crisis may be treated successfully with biperiden.

Abstract: 

Crisis is a kind of response to the risky events and is actually a response to the risk of each event, which is experienced in the form of a trauma and painful situation. Crisis can be limited to a few hours or lasts till many weeks. Crisis tends to activate the powerful reflections in order to help the person in diminishing the problem and brings the person back to the previous stable position in terms of excitement balance. When the person reaches the excitement balance the crisis is removed. The adjective reflections against crisis activate the potential of crisis growth, but the non-adjective responses or reflections result in deepening of the crisis, degeneration repression and appearance of psychological disorders symptoms. There are different types of crisis. Crisis is classified to different types based on factors like, intensity, type of source, nature and the methods of confronting crisis. One of them is crisis caused by natural disasters. The crisis like divorce, loosing a beloved person, internal torture or punishment, stress, loosing properties, homelessness and immigration are among the types that are mostly caused by natural disasters. Psychological effects of accident crisis are lose, guilt- ulcering, stress and migration. Regardless of the type of crisis they have nearly common specifications, symptoms, phases and confrontation methods (intervention methods).In this article we have tried to compile and supply the most important and basic psychological principles and methods strategies of intervention in crisis with the emphasis on the crisis caused by events and accidents, through relying on scientific resources and utilizing clinical experiences. The method of collecting information is library and clinical. We have tried to recognize, discuss and analyze the inter-venture methods in crisis that are in compliance with the society's cultural situation and related to the crisis caused by events and accidents.

Abstract: 

Many experts and analysts in the field of political and economic affairs believe that crisis of Syria not only was domestic struggles, but also was a field for contradiction with benefits of different countries. The aforesaid crisis due to having domestic, national and international complexities is regarded as center of global attention. In fact, Syria is regarded as small world in the Middle East and war in Syria was called as the begin for World War that was began by U.S.A, now the question is that is this war ends with U.S.A as well? As global crisis greatly influences on different countries, the Syria crisis influences on different countries as well. The Syrian crisis leads to going to astray in diplomatic relations of countries, convergence in region, turning old friend to most important enemies and enemies to possible friends in future. American people believe that, America never has permanent friend and enemy, rather the criterion for being friend or enemy is only benefits of America. If during the recent evolution in Middle East except Syria in the remained crisis the global superpowers specially U.S.A was only observer and in order to maintain its appearance and prevent from direct intervention, this issue was not correct for Syrian crisis. In fact, from the first days of Syrian crisis we observed strong participation of such countries specially U.S.A and its allies and Russia. Sending money, weapon, mass media assistance shows field evolutions, dispatching military lieutenants, intelligence service officers, spy satellites over Syria, dispatching terrorist to war of Afghanistan, Alqada terrorist group, Salafi and Wahhabism under supervision of Saudi Arabia government, presence of terrorists from Europe, Asia and terrorists from other African countries are all instances of pressure over people and country that intends to be determined for obtaining its national benefits. Otherwise, the plan of Large Middle East and several wars during recent decades in the Middle East only refers dominance of Israel on world. In fact, dominance over Middle East paves the way for dominance over other countries. Awakening countries and high energy of Islamic countries is regarded as serious impediment for progress of this plan. In this article, it is attempted to study the basics and consequences of intervention of the U.S. on Syrian crisis and some field observations in Syria over Middle East and other regions.

Abstract: 

Introdution: Ischemic mitral regurgitation (IMR) remains one of the most complex and unresolved aspects of ischemic heart disease that the impact of percutaneous coronary intervention (PCI) on improvement of intensity of ischemic mitral regurgitation is not well clarified. Patients with coronary artery diseases and ischemic mitral regurgitation have a worse prognosis than the patients with coronary artery disease (CAD) and those without ischemic mitral regurgitation. We sought to investigate the impact of complete revascularization by percutaneous coronary intervention PCI on improvement of IMR in patients with CAD and comparisons of echocardiography indices in patients with and without improvement of ischemic mitral regurgitation. Methods: In this cross sectional retrospective study, echocardiographic reports in pre-percutaneous coronary intervention time and 12 months after PCI of patients with moderate (≥ 2+) ischemic mitral regurgitation who underwent complete revascularization by percutaneous coronary intervention from Farvardin 1391 to Esfand 1393 were included in the study. Then, echocardiographic data of the patients with improved ischemic mitral regurgitation compared with the patients with non-improved ischemic mitral regurgitation. Results: Comparison of echocardiographic indices before and after percutaneous coronary intervention revealed that after percutaneous coronary intervention (P = 0. 002) in 15 patients (16%), improvement in ischemic mitral regurgitation was occurred. Also, left ventricular ejection fraction (P = 0. 010), left ventricular end-diastolic (P = 0. 003) and end-systolic diameters (P < 0. 001), wall motion score index (P = 0. 003), left atrial area (P = 0. 001) and systolic pulmonary artery pressure (P = 0. 046) in pre-percutaneous coronary intervention echocardiography were different between improved and none-improved IMR group. Conclusion: Although, percutaneous coronary intervention can lead to improvement of ischemic mitral regurgitation but most patients did not show improvement of ischemic mitral regurgitation after percutaneous coronary intervention. The structural abnormality of left ventricle and atrium were different between improved and none-improved IMR group.

Abstract: 

Purpose: The purpose of this article is to present the crisis Intervention pattern from Iranian Counseling Association in Kermanshah earthquake crisis. Methods: The research method is of narrative type. In this research, the earthquake in Kermanshah on 21/8/96 is considered the beginning of the story in the crisis management. Iranian counseling association had a presence in the region for 6 months. The stories in this study were obtained through interviews with locally based members, of which 45 had masters and doctorate degrees. Findings: The crisis management structure of Iranian counseling association was presented at two levels of proprietary performance and public performance. In this process, public performance refers to the level of actors (personality characteristics of counselors) that also influences all of the proprietary function. The proprietary function has three subgroups: a). crises reduction function, b). stabilizing function, c). empowerment function. In each of these functions, the techniques and processes were performed specifically to that function. Actions, unlike themes, are not elements within the narrative, but narratives themselves and the whole narrative is nothing but a sequence of functions. Conclusion: The results of disaster intervention interventions not only help to improve the mental health of the survivors, but also contribute positively to the reduction of physical and psychological problems during post-traumatic events.

Abstract: 

Undoubtedly the Syrian crisis is one of the most important current issues in the international system. This has paved the way for the presence of regional and trans-regional powers in the territory of this country. One of these powers, Russia, has intervened in this crisis in the interest of the Syrian government. Russia‟ s support in terms of political, economic and militaryintelligence was carried out. Of course, the most important aspect of Russia's intervention in the Syrian crisis is direct military involvement in the crisis in the late summer of 2015. Under the influence of Russia's help and assistance, the positions of the Syrian army and its regional allies were strengthened and they were able to regain the control of a significant portion of the areas occupied by oppositions and extremists. In any case, Russia's actions in the Syrian crisis are of a special and unique nature. The question arisen here is “ why Russia has intervened in the Syrian crisis, while it has not intervened in any of the Middle East crises so far? ” The hypothesis is that “ Geopolitical obligations have led Russia to intervene in the interests of the government in the Syrian crisis. ” The research was conducted in an exploratory and empirical way, and is thus loyal to the positivist tradition. ...
